Form 990, audited financial statements, form 1023 and our conflict of interest policy are available to the public.  A copy of these forms will be made available to the public upon request.  A request must be made in writing indicating whether the documents are to be mailed or if the documents will be inspected on site.  If the documents are to be mailed, the request must include the name; address; and phone number of the requesting person; method of delivery; and a check made payable to Relief Nursery in the amount of $15   to cover mailing and copying costs.  A copy of the requested documents will be mailed within ten working days using the method of delivery requested.   If the documents will be inspected on site, the requesting person needs to include dates and times of available and an appointment will be set as soon as is practical; but no later than thirty working days after the request.
